QUESTION: Hi, I suffered a severe crush injury several months ago.  I know have a displaced fat pad(among other thing) on the ball of my foot. The nerves flair up quickly upon walking and become unbearable within a few hundred feet.  Is it possible to get an orthotic device formed for my now deformed foot to relieve the pressure? Thank You
ANSWER: Hi Sandra,

An orthotic device would be an excellent idea.  I would recommend a custom device.  

There are likely other treatments to help relieve your pain.  I perform a simple procedure known as Cryosurgery(see my site below) which helps with nerve related pain.  This may be good for you.

QUESTION: Thank You for the info. Very interesting.  Im sorry I am not in Florida but Northern Ky. Curious though.  This surgery, could it simply remove the fat pad that has shifted over under my big toe and back onto my arch.  Also I dont remember all the nerves that are damaged but it is just about all of them. My right foot was crushed by forklift that drove up on and stopped on my foot.  My foot bust open across the bottom of all my toes.  The docs (podiatrist and neurologist) have said asside from nerve blocks not much left to do, and surgery would likely make it worse. Are doctors afraid of crush injuries?  Seems like it.  Thanks for your input.

Hi Sandra,

It is likely that further foot surgery could make your condition worse.  However, there are some minor procedures other than injections to address the nerve pain.  Cryosurgery for foot nerves and pain has been very successful.  It is minimally invasive and done in the office with local anesthesia.  Even if you did not improve, it would be unlikely that your foot pain would worsen or increase.

As far as the fat pad goes, it is difficult to relocate the fat pad.  That's where an orthotic may be helpful.
