Hi,
I ran a half marathon 2 weeks ago (1:16) and after felt like I had sprained my right ankle (but I have no recollection of spraining it during the race).  With two days rest pain was gone but came back again with first track session.  Repeated rest and then train twice with same result.
After last run, rested ankle for 4 days.  When I tried to run again, on the first step I felt a sharp pain under the ankle towards the heel than propogates forward on the outside of the foot for about one inch.
I have now been icing the ankle / heel area 3 times a day and taking anti-inflamatories, and applying anti-inflammatory cream for 7 days with no improvement at all.

I can put full weight on foot and even "hop" on the foot , I have no numbness,  I walk normally with no pain.  There is no pain when I get up in the morning.  I have no loss of motion or flexibility in the ankle foot area (and can stretch it in various directions).  I can bike hard and really "stamp" on the peddles with no pain.  I can "jog" on tiptoe with no pain (staying on tiptoes).  
The pain only seems to "ignite" and it really hurts, when the foot flexes during the toe off phase of the running motion. This is at a very gentle jogging speed of 10-12km/hr. There is no pain when I land on the foot (heel or ball of the foot strike).
I have no previous ankle injuries.  Shoes are fine, weekly mileage at the time of the injury some 30% lower than previous weeks and body wieght lower that recent months

Any ideas of what is causing this and what I can do?  Many thanks

Hi Peter,

It is possible that you have a stress fracture of the foot or ankle.  In some cases you can have a condition known as a talar dome lesion which can cause pain with certain motions. It is also possible that you have strained or torn a ligament or tendon.  I would see a podiatrist and start with an exam and x-ray and possibly an ultrasound.  If necessary you may need an MRI.
