Have you told your physio that you have increased pain due to the jumping exercises? I'd first talk to him/her and see what their suggestion might be.

On thing that is important when strengthening the back and working with back pain is to work and strengthen core (abdominal) muscles. You might look into good ways to strengthen your core. I think that will help resolve some of your back pain issues.

Hers is a link to some examples:

Some other things you might try in the meantime (while you are strengthening those muscles) are to  apply cool and hot compresses to the area of pain (or ice and heat). Since your pain is chronic, I would alternate the two: 15 mins cold, 15 mins heat etc.

I would avoid the jumping exercises for now and see if your physio can provide an alternative.

Does this help?

If you have any further questions, please don't hesitate to contact me again.

I hope your back gets to feeling better soon!
