Foot/Ankle Pain 3 weeks out from marathon

Question
Out of no where I had a sharp electric type of shock/pain on the inside of my
foot below and slightly behind my ankle bone.  It would come and go just
getting up and walking around.  I was researching tarsal tunnel.  

But then my heel started to feel sore like plantar F.  I've been icing and taking
aleeve/advil.  Took a few days off from training and went out for a easy
longer run on sunday.  Started out slow and it felt good.  Picked it up in by
mile 8 I started getting some foot/ankle pain on the outside (opposite side)
which within a few strides brought be to a stop.  I stretched, walked a bit and
was able to continue.  But I still feel something walking around on the foot
and heel.  The pain is around where the foot and angle meet on the outside.  
Now I'm really confused.  I'm going to be on the road on business and can't
get into a doc but when I return I'm not sure if I should see my podiatrist (I
have orthodics) or a PT or ortho.  Any ideas?  Thanks

Hi Tom,

There are a few possible diagnoses.  If you are having pain in the heel, you may have abnormality or compression of the medial calcaneal nerve which comes from behin the ankle and goes into the heel.  This is a fairly common problem and often misdiagnosed as plantar fasciitis or tarsal tunnel syndrome.  

Also, after extensive activity such as a marathon you can get impingment of the nerves known as the sural and superficial peroneal which are on the outside or lateral part of the ankle.  Sounds like you have symptoms of overuse.  However, I would see a Podiatrist because the longer these nerve problems linger, the harder they are to treat.  I would see if you can get a solid diagnosis and possibly consider physical therapy as well.
