Middle back pain, stiff neck/shoulder w/ headache

Question
To start, about 2-3 years ago i had a snowboarding injury where i fell and hurt my middle back to the point that i couldnt walk for a week. Any time i tried to use my left leg it sent a shock through my back. Doctors said they couldnt see anything from X rays.  
This injury hasnt bothered me since, until recently (not sure if its related). Abouth 3 months ago i moved into a new place and did a lot of heavy lifting by myself (washer/dryer/kng bed etc...). When i finished my back hurt right in middle really bad. Bad enough that i had to lay down. About a day and a half later i started to get headaches in the back of my head that hurt pretty bad. I went to a family doctor and he did some basic checks for anything serious, but nothing looked abnormal, so he sent me home w/ some decongestants and antibiotics (sinus infection stuff). That didnt really fix it, but it has gradually gone away. I havent taken any pain killers for it for a long time. Some physical activity (golf, work, waterskiing) will cause the pain to come back a little. I went to a chiropractor as well and it didnt help much. Most of the time i dont even notice a headache, but if i nod my head sort of aggressively i can feel a slight headache in the back of my head. I also have had middle back pain at night when i sleep, if i sleep flat on my back its ok. I have wondered if i have a herniated disc causing the pain and headaches.  Please give me some advice, i have been really healthy and recently had a physical (i am 25) and everything check out good, so this headache/back issue really bothers me. Please let me know what it could be or what i should do.

Cool Name!

You are definitely on the right track, an MRI would definitely be advisable. A Chiropractor isn't enough if you have a herniated disc, this would also explain the lack of results with standard manipulation. A DC that specializes in towel traction would be needed if your hunch is correct, I believe it is very possible. Get the MRI and I will refer you to someone with the skills necessary to fix it.
