Question
I have had an uss on my heel and as well as plantar fascia they have discovered a thickenend tibialis posterior tendon and an elongated irregular intrasubstance fluid collection. That states it represents myxoid degenerative change and intrasubstance tear! What on earth are they talking about and will it need surgery to repair it in your opinion. my Gp is requesting an orthopdic opinion because the Gp has no idea what it is either. Thanks

That said the answer is...
Usually with plantar fasciitis there is a low or high arch. Sounds like your arch may be low. The tendon that raises the arch is the tibialis posterior tendon. When this tendon gets pulled and has small tears over time-then the tendon shows degenerative changes that are mentioned above. The changes in the tendon do not necessarily need surgical treatment. Sometimes physical therapy, weight loss, decrease inexcessive physical activity  and orthotics may help.
If you have an extremely low arch, then surgical intervention may be The findings indicate possible arch dysfunction.
