Question
Hello Mr. Ryan.
Iam a 50 year old male, who has for the past 8 years suffered with back problems, and still have chronic pain in my lower back.  I have had 2 surgerys. Laminectomy/diskectomy at L5 - S1, and a spinal fushion.
I recently had another MRI, and I need help with the readings.  they are as follows:
Findings:
A combination of scarring and spondylosis is demonstrated at L5-L1.  This results in mild to moderate thecal sac effactment the left of midline.  Distinct evidence for recurrent disc herniation is not identified.  Rest of the lumbar levels appear unremarkable without evidence for discogenic change.  Relative good maintenance of vertebral body heights and alignment is demonstrated.  There is no evidence for central canal stenosis or neuroforaminal encroachment.
Impression:  Chronic changes at L5-S1 are associated with bone hypertrophy and disc narrowing.  Minimal enhancement is seen consistent with scarring. This results in mild to moderate left paracentral thecal sac effacement.

I dont know what much of this means.  Can you assist me?

Answer
The MRI results show typical scarring and disc degeneration in the area but no evidence of anything new or recurring.  The only point of interest would be the mild to moderate pressure against the spinal cord sac which can cause pain in some patients and not in others.  Your doctor would be the best person to give you the information regarding how the MRI results related to your particular case.  He/she has all your films and medical records.
