I am 40 years old and i had a fusion of my L4 and L5 after fracturing my facet joint 11 years ago and now i am starting to have a lot of pain in my back again and pain going down my legs. I'm on diclofenac, tramadol, gabapentin and paracetamol. Have you any idea why i am starting to have the pain again

We need more information.  Where is it specifically going down your legs?  Is it positional (Does it hurt with certain motions) With a fusion, there is a lot of stress placed on the segment above.  Please follow up with me.
