Question
pls advise on no drug alternatives for extreme pain from piriformis/hamstring in spasm.  I sustained this in sitting improperly while in a foot/leg cast.

I feel for you! piriformis pain can be extremely painful.

There are two very common approaches (non drug) to relieve pain in piriformis/hamstring area.
One is receiving massage therapy from a licensed/certified massage therapist. A massage therapy can help relax muscles and help minimize or eliminate spasm in the area. It will likely take more than one session and then perhaps followup sessions depending upon the injury.

Another common approach is chiropractic. Since you were "sitting improperly" while in a foot/leg cast, it is likely that your hips are out of alignment & a chiropractor can help but them back in alignment plus make adjustments that will relieve/release the piriformis.

As a licensed massage therapist, I will say that I have seen the best results (and quickest) using a combination of the two approaches. If you do use both, try to schedule the appointments in close proximity (ideally within 2 days).

Acupuncture may also help. Also a personal trainer can help strengthen muscles opposing the area & help you stretch the piriformis/hamstring area out.

There are some great hip stretches and techniques you can use at home for piriformis release as well as hamstring release. There are many videos on youtube that demonstrate some of those. If you would like I can give you links of ones I recommend.
