Question
I am a 51 year old female. I am currently going to chiropractor since 9/5/08. I was diagnosed for acute moderate spasm, L3 suluxation and marked L5 S1 degeneration of disc. Lately I've been waking up with tightness on right buttock in cheek area. I want to know what you think it is. If I use shower massage in shower on buttock or moist heat, it feels alittle better. What do you think? Thanks

Sorry to hear of your low back problems, pain/ tightness in the Right Buttock.

As I answer this question it is 9- 18- 08.  Your question says you initiated care on 9- 5- 08.  You have been under care for just under two weeks.

As a Doctor of Chiropractic, I have my bias, and I admit my bias:
Congratulations on going to the Chiropractor for a back problem!
Many people take medication (whether over-the-counter (OTC) or prescription) which is often ineffective, and sometimes unsafe- see info later- for their pain.  I think you are in the right place.  It may take some time for your body to heal, even with the correct treatment.

This Staten Island Chiropractor (me) sees back problems all day every day.  Chiropractors are often called the 'spine specialist' by lay people.  I will describe what is relevant to your condition.

 The course of VSC is highly variable.   Some patients with VSC literally cannot walk, yet other patients with similar test findings may be able to run marathons or lift heavy weights.    Some patients immediately develop symptoms related to the VSC,  some patients take years to develop symptoms.  Some people suffer for only a few days with pain and symptoms, some people suffer for months.  Some people recover in days, some take months or years, depending upon severity of the condition.

         Improper spinal function can choke, stretch, or irritate delicate nerve tissue. The resulting nerve system dysfunction can cause symptoms elsewhere in the body.

    Histopathology:
         A rise in temperature from an increase in blood and lymph supplies result in swelling and inflammation. Discs can bulge, herniate, tear, or degenerate. Other soft tissues may suffer permanent damage.

    Pathophysiology:
         The VSC contributes to OsteoArthritic degeneration.  Bone spurs and other abnormal bony growths attempt to fuse malfunctioning spinal joints. This spinal decay, scar tissue, and long-term nerve dysfunction can cause other systems of the body to malfunction.

Degenerative   Disk    Disease               (  DDD,   DJD,  OA )

Degenerative Disc Disease  (old term =  DJD) is  actually  a  quite  common  condition.   However,  the course of the condition is highly variable.   Some patients with DJD literally cannot walk, yet other patients with similar X-ray/ MRI findings may be able to run marathons or lift heavy weights.   Some people suffer for only a few days with pain and symptoms, some people suffer for months. Although the course is highly variable, there are certain steps that are always helpful to follow:  Ice the area when painful-  10/ 15minutes on, 30/ 45 minutes off;  avoid certain postures and movements;  bend the knees when coughing or sneezing;  bend the knees when lifting anything;  do not extend legs straight out when sitting, laying down, lifting, or driving.   Avoiding certain movements and postures will prevent aggravation of the condition.   Practicing good postures and movement patterns will help the condition heal and be less painful.  Degenerative disk disease,  degenerative joint disease,  deteriorating discs (disk)   is/ are  often caused by,  or often  concurrent with the Vertebral Subluxation Complex.

what do I think?
         Well, if it seems to help, use it!

I usually prefer ice.  Most people do not do anything for health maintenance, but will do something for PAIN.  For PAIN, I prefer to use ICE.  When the pain is gone, THEN I prefer to use moist heat for the stiffness that is often left after pain is gone, while progressing towards better function.  Heat can sometimes backfire when used on PAIN.  Pain is an indicator of inflammation.  Inflammation creates it's own heat, there is no need to add more fuel to the fire.  PAIN = use ICE.  Stiffness = moist heat is usually okay.

P.R.I.C.E. =    P  protection.   R rest.   I  ice.   
C  compression.   E  elevation.

Hurt  yourself ??  injury/ trauma/  bruise/ not even sure - - a broken bone ???????

You should certainly get to the Doctor or trained health professional to be examined  if you think it  is  bad  enough.     BUT  in  the  meantime,  some  simple  steps  may  help.   

PRICE.


The  PRICE  protocol will usually help just about any musculoskeletal injury.

P = Protection;  protect the area from further injury- stop what you are doing, can you pad  or bandage the area ? Tape the area? Do something to protect the area !

R = Rest;  rest the area as much as possible, if a part is damaged, give the body time to repair the area

I = Ice;  ice is a potent anti-inflammatory; it will slow swelling, help with pain control (ice is analgesic), place ice over cloth over the injured area for 5, 10 minutes, off the  injured area for 30 minutes; then back on again

C = Compression;  to help prevent swelling compression may be helpful- for example if it is an ankle, wrap the ankle with an ace bandage and ice the area.  Many body parts may not be applicable to compression.

E = Elevation; again to fight swelling and pain, raising the injured body part above the heart may be helpful- ie raise an ankle or knee, you lay down, raise the injured part

PRICE  may  help :    Sprains,  Strains,  musculo-skeletal injury,  bruise,  contusion

Sometimes  a slip, trip, fall, heavy lift, athletic injury, or a motor vehicle accident can cause a bump or bruise, sprain or strain.  Sometimes also involved with the injury is a vertebral subluxation (or other joint subluxation) (subluxation is a mechanical problem- of any joint- and requires a mechanical correction).  In the spine, a sprain/strain  is a misalignment,  a 慿ink?in the joint, a stretch of the muscles, tendons and/ or ligaments:- more properly termed a Vertebral Subluxation.   Sometimes symptoms can come immediately, sometimes days, weeks or even months later!- Well after the actual trauma !!!

Again, as stated previously, most patients are compliant with their doctors when they are in  >PAIN<.  Once pain diminishes or disappears, the patient often forgets about the doctor, the appointments, the recommendations.

A warning on  > PAIN < :

PAIN     Pain  is a  憆ed light on the dashboard?  Pain is your body telling you that something is wrong.  That RED LIGHT on the dashboard of your car- -- do you ignore it until the car breaks down, or do you get it checked and correct the problem?  Pain, pain in your body- -- do you ignore it until your body breaks down, or do you get it checked and correct the problem?   Pain,  do you cover up the pain by taking a painkiller?  Take a painkiller, mask the pain, and allow a problem to progress in your body?  

NOTICE  ON  PAIN  RELIEVERS:  Label changes ORDERED by FDA;  the FDA announced proposed label changes for OTC over-the-counter pain relievers to include the potential for stomach bleeding and liver damage (FDA news  206- 207;  12-9-06) ;   

The  American Heart Association issued a scientific statement recommending medical doctors change the way they prescribe OTC pain relievers from a first choice to an alternate of recommending non-pharmacologic treatment (AHA statement  2-26-07).

You have been seeing the doctor for almost two weeks.  Chiropractic works WITH the body to heal naturally.  I do not know how long it took your body to get into it's current painful condition through injury and degeneration;  now you must allow the body (and the doctor) time to heal itself.

                                  Treatment   Regimen

Why do I need to have treatment __________________ ? ! ??
( TWICE A DAY,  EVERY DAY,  3- 4x/week, 2- 3x/week, 1- 2x/week )     Depending upon your specific condition, your specific case, you may need to be seen many times in a short period.  If your condition is severe, complicated, particularly painful; you may need close, frequent attention.  Similar to medicine, usually the more severe the case the more often medicine is taken with larger doses.  The Doctor of Chiropractic works with hands and therapy in person to accomplish the goal, the MD can treat 憀ong distance?via medicine without actually seeing the patient for treatment.

Why do I need to have treatment for  (weeks,  months,  years!?! )  ?     Compare  for a  moment:   chiropractic  treatment,   medical  treatment,   dental  treatment:   very  similar !!!
Again, the Doctor using chiropractic techniques needs to get 慼ands on?in person.   An MD can treat a patient without actually physically seeing the patient every day through medicine;  i.e.:   搮Here are 60 pills, take one a day every day, and I will see you in two months厰.   The dentist can say- 搮here, wear these braces 24 hours a day, 7 days a week, for the next two- three years.  I will see you every two weeks, but I抣l be treating you every day, 7 days a week, 24 hours a day for the next few years厰 .

People can wear braces and retainers for YEARS.  It can take  YEARS  to move the bones (known as teeth) into their proper position.  It can take years and years, and thousands of dollars !  But, those teeth (bones) have to be properly aligned to have a nice smile.  BUT you do not have to keep wearing the brace or retainer, you can take the brace off any time,,,, and let the teeth go back to their misaligned positions (habit).  You can do as you wish.  On/ off, go for treatment, not go;  listen to the Doc or not.

Some people with certain conditions have to take a medicine every day, seven days a week, 365 days a year;  搮..for the rest of their life??.   Sometimes someone can have a damaged heart, lung, liver, kidney, pancreas;   and they have to take a medicine, sometimes a FEW medicines, every day, sometimes for years,  sometimes 搮for the rest of their life厰 .   Similarly, a damaged neck or back, a damaged spine, a damaged nervous system can sometimes use treatment  ?..for the rest of their life厰 .

Most people who go to the Doctor of Chiropractic feel a difference in their pain in as little as two or three weeks.  Instead of taking years (like braces and the teeth) to make changes, the dynamic spine can start to feel better very quickly.  To maintain the proper position and movement of the vertebrae (bones) it can take a continued regimen of care (of course not as frequent as the initial schedule, but to maintain gains, maintenance has to be performed).  BUT you do not have to keep up with treatment, you can stop any time you want,,,, and let the vertebrae go back to their misaligned positions (habit).

It is a joke amongst Chiropractors that the DC is a magician-  first Chiropractic makes the pain disappear, and then the patient disappears as well !!,  BUT- pain is the last thing that comes in a problem, and the first thing that disappears under treatment,,,, but the disappearance of pain does NOT mean correction of the underlying health problem.

I am sure you have given your DC a case history consultation, had a physical examination, probably had Xrays, and could possibly progress towards having an MRI of the painful area(s).  These are steps in care I assume you have had.
