Shoulder popping and grinding

Question
Hello.
My boyfriend has had some pretty awful pain for the past 5 years and refuses to go to the doctor or ask anyone's opinion.

The pain is in his left shoulder blade and shoots down his arm into his hands. Whenever the shoulder is pushed on it makes a loud cracking noise in his wrist and fingers. When he moves his arm up and down it makes a grinding noise. It causes him a great deal of pain and half the time cannot sleep at night it hurts so bad.
I try and massage it as much as I can, but whenever I rub the shoulder it makes loud cracking and popping noises all down his left side.
He screams sometimes it hurts so bad, and he is not the kind of guy to complain about aches and pains. It gets better like when he's at work or doing something actively but as soon as he slows down it gets way worse, especially at bed time.
The problem is getting worse and worse every day and I'm afraid if we don't figure out what it is he'll soon be disabled due to his stubborness.
Any ideas on what it may be?

he needs an xray and possibly an mri. If he is unwilling to go to a doctor  then there is nothing you can do. Even if i did know what was wrong with him, he still needs to see a doctor. He needs to realize it effects his quality of life and that he has a problem , then he will go. good luck! his problem could be a number of different things and i couldn't possibly diagnose him without any diagnostic tests.
