horrible facial pain and itchy arms in the fall

I am at a loss as to who to see for help.  For the last 4 years my arms would itch to where I would claw at them and they would bleed.  I saw a dermatologist, no answer.  I researched it myself and my symptoms sounds like that of brachioradial pruritus.   Last year, same time, I started experiencing extreme facial pain that would wax and wane.  I went to my dentist and oral surgeon, had countless xrays ro rule out anything with my teeth.  They said all was fine.  I saw a ENT and he said it was my teeth.....I had a ct scan and it showed sinus problems.  Pain was unbearable, would come on all of a sudden, extreme pain that would radiate up to my right eye, lower jaw and upper teeth on the right hand side.  Eating or drinking cold would not trigger the pain.  I would be fine and then all of a sudden the pain would come on....last for 15 minutes then subside and go away.  This cycle would go on all day long and into the night, waking me up with pain.  Doctors put me on antibiotic.....nothing.....put me on hydrocodone for pain....nothing....pain was still there.  I went to a neurologist at the Mayo clinic in Jacksonville, FL for a mri.  He ruled out trigeminal neuralgia and really didn't know what was going on.  The same visit, I saw an orthopedic specialist for carpal tunnel that came on all of a sudden.....numbness and tingling in arms and hands. I saw a ENT specialist also who did a procedure where he put a scope down my nasal passage.  He said I have a deviated septum and a nasal spur.  He was unable to go down the right side of my nose because of the spur and deviated septum.  He does not know if this is causing the facial pain.  My neurologist called me to tell me that my b-12 was extremely low and might be causing the facial pain.  I started b-12 injections weekly and that seemed to give me relief from the pain.  Pain ended last March, have not had a b-12 injection since May.  I thought, great.....the pain from last year is gone and over.  Then, 3 weeks ago, my arms started the itching again and then the facial pain started back.  The pain is horrible, unbearable....and what is worst is not knowing what causes this or how to get relief from it.  Trying to teach full time and raise 5 kids is hard when I am in horrible that that causes blinding headaches and unbearable pain.  Where do I go from here?  I am willing to go about anywhere to get answers.  Thank you for your help.  Oh, one more thing..approximately 12 years ago, I experience horrible neck pain to where I couldn't turn my head...this was the first time, and it lasted for a couple of days.  Went to chiropractor, ordered an xray, that showed I had whiplash....a twisted vertebra...he did some therapy on my neck and then the pain was better.

It is a good sign that you had some relief after going to the chiro. Let me ask you this, is this a general chiropractor or a specialist?
If you had any X-rays or MRI taken can you send me a copy via email or physical mail?  This way I can provide you with a better answer after seeing what sort of shape you've gotten yourself into.
