QUESTION: hello dr. timthy!
hope you are fine. as i mention in last mail that i have stop tking medicines as you advise me for sciatica pain.
my problem i can not walk/stand for more then 10min.
i start feeling pain in my buttock and leg.
hope you will answer my question. i am in great trouble.
should i continue hot bag?
should i keep stretching my leg? which i am not doing now...

ANSWER: Last time I gave you excellent advice, you gave me the worst rating I ever received, after you re-did the rating, you still gave me a bad rating with 8;s in two categories.

If you look what other people have done, you would notice that my ratings are all 10's, except for a couple 9's and a 8. Where you get off by giving me a 5 for timeliness after answering your question within a half hour tells me you are here to start trouble. Please ask someone else.

QUESTION: Dear Dr.Timothy Dunin!
hope you are fine.
My sciatica pain is confusing me.
I have stop taking medicines.I am using HOT bag alomst 3 times a day.My blood test is normal.
Numbness in the left is recovered 95%.
Pain in the leg is recovered 85%.
My problem when i am sitting i feel minor pain in Left buttock at the point where the whole weight of body depends.

I still cannot stand straight. Cannot walk long. After a small walk i start feeling BURNING pain in my Buttockand NO MORE in the leg.

This pain dont let me walk and stand straight.so i need to bend my body.during walk of 10 min i need to take rest in between.some time i also feel (after walk) stress from my left back to left shoulder.which disappear after taking rest.
some time i feel "crams"in the morning in my left leg which i try to avoid as it may results in  more pain.
I am using muscular menthol cream on the buttock now a days.

QUESTION:
Should I continue medicines?
Should I use Waste Support Belt?
Should I keep streching my leg(which i am avoiding now)?
Should I contiNue Hot Bag?
Should I practice some exercises?
Should I avoid Walk (as it result in pain)?

I don't no why it was so hard to give me 10's and a yes like everyone else, the info I gave you took less than
50 minutes to recieve and yet you gave me a 5 for timeliness. Yes you did change it, up to 10's and 9's with a NO.

Not exactly a ringing endorsement. I am not sure why you would do that after I spent a great deal of my free time trying to help you. You need to contact allexperts and have them fix it. Then I will help.

Adding 10's to an already molested score doesn't erase the damage. You have to ask allexperts to take it taken off.

In the mean time, I will help you half way, stop using heat and use ice, the heat is making you worse.

I will tell you how to get even better once those bogus scores are removed.

Now to your questions,

Should I keep stretching my leg(which i am avoiding now)? Depends what you mean by "stretching". The only thing you should be doing is icing it down 3x/day for 15 minutes and laying on your back pulling your knee to your chest 20X/each side then both knees simultaneously 20X/ 5X/day, especially before you go to bed and before you get out of bed. More stretches and exercises will be forthcoming after you are 95%.

Should I continue Hot Bag? Absolutely NOT! If something hurts, it is usually red, hot, swollen and inflamed, the last thing you want to do is introduce more heat in that environment. Just look at any professional athlete, when they get hurt, what do they use?
Should I practice some exercises? Only the ones I described.
Should I avoid Walk (as it result in pain)? No, walk until it starts to hurt, then stop and ice it down.

Find a D.C. that does F/D, you need it, home care alone isn't adequate, you need a professional to give you that added push. My recommendations are fine for a simple problem, if you have a herniated disc or degenerative condition, only a D.O. or D.C. can fix it.

Let me know when you find one and I'll check him out to see if he is competent.

Without examining you, it is impossible to tell if it is your SI or Low back.

A D.O. is a doctor of Osteopathy and a D.C. is a doctor of Chiropractic.

You need to follow up with one of thee guys to get an answer to the SI question and to get completely fixed.

i would like to add some thing more in my current condition:

as you know earlier due to sciatica pain i use to walk w/ little bend
in my body as i was not able to put on my left leg.

NOW i have notice my body structure is changed.
I AM HAVING BEDN IN MY BODY WILE STANDING AND WALKING.
MY UPPER BODY IS NOT ALINE W/ WASTE. GOING TOWARDS RIGHT.

i cannot make it aline while standing . i have tried making it aline by
layig down,on my belly.
i am asking my partner to stand on my waste by putting pressure on
sacroillac joint.its pain full to my left leg and waste .

QUESTION:
HAVING CHANGE IN BODY STRUCTURE DUE TO SCIATICA is normal?
IS THERE ANYTHING SERIOUS?
PROCEDURE I AM DOING NOW TO ALINE MY BODY STRUCTURE IS OK?

ANSWER: Hi aufan,

If by, "change of structure" you mean your leg muscle is shrinking up, then you need to go to the Dr. immediately.

This is denervation atrophy secondary to nerve root compression. If you can walk on your heels and toes than that's good.

Explain what you mean.

ther eis no problem in walking on toes or heel.. just little tension on my left leg.
change of posture:
as i am walking w/out putting weight on my left leg that's why my waste is bend in the same direction.

when i try to straight it by laying down  (by using the procedure i explain earlier) i feel tension in the left leg an dbuttock. but by doing that i feel more relax also.
tell me if iam goingin right direction or not...?

ANSWER: Hi aufan,

Does it hurt to cough or sneeze, if it does, does it shoot down your leg?

You are describing what doctors refer to as antalgia. You are leaning away from the side that hurts. When you put pressure on the left side it probably goes into your buttocks and into the outside of your calf.

All I can do is give you my best guess as to what is wrong. I can't effectively tell or show you how to self treat without examining you in person. Is it impossible to see a Dr.?

Once you have a concrete diagnosis, I can help you immensely, right now I could harm you by suggesting otherwise. You could even have an aggressive lesion next to your spinal cord or infection in the disc. It would be crazy to supposize further until you are examined by a professional IN PERSON!

I can't stress this enough, please take my advice and see a Dr., any doctor.

I need to know that all other possible diagnoses's have been ruled out.

It won't cost you much for a simple evaluation.

Get back to me when you have been examined.

QUESTION: dear Dr. tim,
i followed your advise and went to my Dr. he again suggest me same muscular relaxant and said nothing to worry!!!

Recently i have started "Yoga exercise" and its realyy help fully.
Now i dont feel pain in my leg when i am sitting.
I feel more flexibility in my leg as i can touch my knee to my face when lying down.....
infact i feel much better now....the bend in my upper body part is also improve.
the only thing which is disturbing me is I cannot stand and walk for long time....when iput stress on my leg by standing or walking i start feeling pain but not that much worst as it use to be .....
like to ask:

where am i ?
when i will be able to walk normal?
should i put stress by walking or standing in this condition?

Hi Aufan,

You probably can't walk for long because you have a possible "slipped" disc. You might also have a short leg undetectable without standing or weight bearing X-rays,

I like the fact Yoga has improved your flexibility, this is very important. It can only do too much, if you have a disc problem, you have to get F/D to fix it. Only a D.C. can do it. Did you find a D.C.? It sounds like you are symptomatically 90% better but biomechanically 50% better.

Getting a MRI would be ideal but I sense you are short of funds or the Dr. won't order it. Keep trying, without more info, it is impossible to tell.

More than likley, walking is the best thing you can do for a back problem, without an MRI ruling out other pathology, it is impossible to know for sure. Keep trying to get one!
